Abstract

Antibiotic-resistant endodontic biofilms represent a major challenge in achieving predictable root canal disinfection and long-term treatment success. Conventional antimicrobial approaches often fail to completely eliminate complex microbial communities due to limited penetration, biofilm protection mechanisms, and potential damage to surrounding regenerative tissues. Smart nanoparticles have emerged as innovative therapeutic platforms capable of providing targeted antimicrobial activity while preserving the viability and function of dental stem cells. These advanced nanomaterials can be engineered with responsive properties, enabling selective activation within bacterial microenvironments through mechanisms such as pH sensitivity, enzymatic recognition, reactive oxygen species generation, and controlled drug release. Their ability to penetrate biofilm structures, disrupt resistant microorganisms, and minimize host cell toxicity offers new possibilities for precision-based endodontic therapy. Furthermore, integration with regenerative materials may support tissue repair and improve periapical healing outcomes. This approach represents a shift from broad-spectrum antimicrobial strategies toward intelligent, biologically compatible infection control systems. Future developments in nanoparticle design, safety evaluation, and clinical translation may establish smart nanotechnology as a valuable tool for managing persistent endodontic infections while maintaining the regenerative potential of dental tissues.
